What would happen if I didn't go to jury duty?

For whatever reason, the government is stupid enough to not have my current address, even though I've had a drivers license in a different state, no less, for 7 years. My dad informed me that he received a jury summons for me for the end of the month. He told me he'll fill out an exemption thing for me. Really? Would they really hold me accountable for that? Would I have an outstanding warrant every time I went to visit my home state?
