Some national dentist study actually advised against the removal of wisdom teeth before they cause problems. Since your wisdom tooth is decayed, YES, get it removed as soon as possible. It might be painful depending on your dentist and how you react to anesthetic. If you are put to sleep for the surgery, you obviously won't feel a thing until you wake up. Then you take painkillers. For me, local anesthetic was ineffective and it was (literally) bloody painful.
